  • Vaccine Exemption Inquiry

    Here is my inquiry into the vaccine exemption protocol, I sent the following inquiry and the departments response goes as follows:

    Vaccine Team:

    For the Religious and Medical exemptions, please provide the form and or process the Department is requiring for approval of said exemption ASAP in order for me to inform my members of how to go about the process.

    Elena, hello! I’m so glad you reached out on behalf of your members.

    At this time, we know the process for these exemptions will be forthcoming from DAS. We do not yet have details around how this will be accomplished, nor do we have the actual forms, however we expect to see more information on this soon – it’s a very common question that DAS CHRO is getting and I know folks are anxious for it.

    As soon as we receive further guidance, we will ensure we communicate with all employees. In the meantime, we very much appreciate your patience and your leadership.


    Thank you for the response. This is a time sensitive piece of info my members need and would like to have had prior to the announcement of the Governor’s mandate. There needs to be enough time for my membership to be able to accomplish this task should it be a position they need to take. I would like to see and review the form and or process as soon as it comes in.
